Two Options for Ordering and Payment
LifeCrossings is a provider of educational resources for churches and other non-profits. Churches, other non-profits, and individuals may use either option 1 or 2 below for payment.
Option 1 - Pay by check
Please use the following procedure:
(1) Click "Order Form - Pay by Check." To print this form, click "Fullscreen" (near Scribd just above the order form) then choose print. If you wish to return to the website menu after printing, click "Exit Fullscreen".
(2) Fill out the order form.
(3) Ohio locations only: Enclose a copy of the tax-exemption certificate for your church or non-profit.
(4) Add the applicable shipping cost to the cost of books ordered.
(5) Enclose a check for the total amount.
(6) Mail the completed form, tax-exemption certificate (Ohio locations only), and payment to: LifeCrossings, LLC, 4384 Kenmont Place, Columbus, OH 43220-4059
Option 2 - Pay via PayPal
(1) Click on "Order - PayPal" option.
(2) Complete the order page.
(3) Use either a credit card or PayPal account for the purchase.
(4) Provide the information required on the PayPal page.
(5) Ohio locations only: When PayPal returns you to the LifeCrossings website, click to "Contact Us" and use the "Comment" box to submit the name of your church or other non-profit, address, email address, and tax-exemption certificate number.
We cannot ship orders for Ohio non-profits until we receive the tax-exemption certificate number.
